Enrollment Agreement

There is a one-time registration fee of $250.00 and an annual enrollment fee of $100.00 that must be paid along with the first week’s tuition in order to reserve a spot for your child at the Center. This non-refundable payment ensures a space will be available for your child on the agreed-upon start date.

Expectant parents are asked to confirm a two-week window for a start date, and current parents are asked to give an exact start date for their child.

Weekly Rates: Please CHOOSE which rate applies to your child(ren) or write in the previously agreed upon amount below the table. 

Enrollment Agreement Form

By signing below, I acknowledge the enrollment policies and procedures outlined in the Parent Handbook (including payment days, late fees, and late pickup) and agree to adhere to them. I agree that if I, for any reason, decide to depart from Bright Eyes, I will give 2 weeks' notice to the Center.


Should Bright Eyes have to collect on my account due to delinquency, I acknowledge that Bright Eyes will be entitled to receive court costs, reasonable attorneys' fees, and/or a collection fee of 25% of the balance due.

agree to pay the enrollment and registration fees as well as first weeks tuition (non-refundable fees) in order to reserve a spot for my child(ren) at Bright Eyes.


I understand that my child can only attend on

(days of the week) and that any extra days must be preapproved and will incur an additional cost.
